Personally - I would only dose up with a biocide if there was evidence of contamination. But I would examine the fuel filter on change even if that meant I had to hacksaw it through to conduct the examination. If I found black spores or gunge, I would then run the tank as low as possible, drop...

It is difficult that one because this sort of contamination is blind to how new your car is per se. It only needs one contaminated fuel to kick off the process. To balance that do not get too paranoid - just inspect fuel strainers carefully, post change, for any evidence of 'black gunge'.
