Annoying little jerk - W210 220CDI

Good Morning Ladies and Gents.
I wonder if any one can help?
I have an E class W210 220CDI with 128000 miles on the clock.
Recently when the car is coming to a halt just at the moment of coming to a standstill the car gives a small jerk forward and a small puff of black smoke is emitted from the exhaust pipe.
Has anyone experienced symptoms such as these or any clues as to how to repair this problem?
