C250 auto holding 2nd gear

Just wondered if anyone has had this before.
The car has done it occasionly since i've had it but it seems to be getting worse.
Sometimes on hard acceleration, especially after a cold start and going uphill, it will hold 2nd gear. It will accelerate through 1st and 2nd but wont change to 3rd. Once the throttle is let off and the speed comes right down it will eventually let go of 2nd gear and go to 3rd.
It has done this to me loads of times from a standing start on traffic lights - its really embarrasing.
The gearbox has been checked and shows no faults.
Any ideas??

I am not sure what the situation is with diesel 202s but with petrols they will delay upshifts when the engine is cold. This is perfectly normal. If I recall correctly this is to warm up the catalytic converter.

this does it all the time, mainly on hard acceleration.
Also, the kickdown has never worked.

Is it a cable operated throttle and kickdown on that model?

Stretched/out of adjustment cables??

Its cable to the variable resistor, the kickdown is a switch at the bottom of the pedal - which i have tested and does meter out ok.
