Outside of the cam chain issues which as you say are well documented I’ve not read of anything internal issue wise in my 12+ years with an M271 (& on forums ) .
Regular oil changes is probably the best M271 advice .
150k + ? , the car will fall apart around the engine before then .

this is what I tell everybody. changing the oil and filter is the most important thing you can do to an engine, and forget what some manufactures tell you !!!! all depends on what mileage you do but twice a year is a good idea.
before i bought mine i seen some with more than a 150K so they can do it, and mines only half way their

Got 80K on my 2003 CLK, have not done the timing chain as yet, doing approx. 3.5 K per year and change the oil/filter every year. Not sure if I will ever do the chain, what's the cost guys at a decent independent? Could do it myself but really cant be ****d at my time of life.

Which would near write mine off. If it's not the chain then the subframe will finish it off and it will owe me nothing after 10 years of ownership. If it passes the MOT next month then it will be 11 years and so on. Amazing that a car so completely rust free externally and perfectly functional can be worth so little.

Had my 2004 203 M271 supercharged at 143k, no chain issues. The car never asked for anything outside routine service items.
Different story with my S204 M271 turbocharged, had dreaded timing chain at 36k FMBSH.
Probably combination of maintenance and luck that ends up with the stretched timing chain...

A lot depends on how the M271 has been maintained.
At 95,000 miles I needed to change camshaft sprockets, timing-chain, guides and hydraulic tensioner.
That was 8,000 miles ago.
Since then the engine has been good and shows no sign of any further problems.
It gets regular oil and filter changes with correct grade of oil.
Before I got the car (the W203 C200 SE Coupe - Year 2005), the car had been poorly maintained in so-many ways.
When it's going well, it's a great little car.
However - Overall - It's the most troublesome car I have ever owned.
