Hii,

I own my 1991 w124 300D estate for 5 years. The muffler (rear end) is still the factory fitted original one. There are just a few small holes due to corrodion. Other than that no problem externally.

Does it need to be replaced? May it be blocked internally? Does it affect performance of the car ?

Don't think it will affect the car performance. The likely failure point in the future will be somewhere on the pipe from the mid silencer to the rear box,often at the weld where it meets the rear silencer. This is often due to exhaust condensation on the inside.
Holes in the exhaust in the UK mean an MOT fail ( annual test for vehicle roadworthiness after 3 years)
