W211 e220 engine rpm fluctuating

So, after work I started my car and noticed engine rpm going up and down rapidly. Tried to drive, the car go very slowly. Driven to the next driving spot and left it there. Feels like one or two cylinders not working. Check engine come up on a dash. Scanned with cheap obd scanner showed p0404 error. I checked this is egr related, but I don't think this is the real problem here. Car puts a lot of smoke and hardly moves. Any ideas what could be the issue?

I was getting 404 code. Cleaned the EGR but didn’t help. Needed a new one. Easy to clean with some oven cleaner so worth a go. I loosened mine with an old chisel (very carefully) as it stuck in good and proper.
 
Thanks for replies, I took the car to the garage they took out the egr valve, checked the electrical part which seems working and cleaned the soot. They say there is another issue there, possibly a jam in exhaust pipe. Could be catalytic converter cloged up. I will update when anything gets clear. Car has just 100k miles.
 
Just took the car from the service, and the egr cleaning solved the issue. There was not any more problems. At first it still showed egr valve fault on dash, but after some driving it dissapeared and now running fine.
