Leave it in D, press parking brake, switch off ESP, go to service menu choose Dino mode and at the first site of yellow release the parking brake and floor it.
I changed my mind I've decided that I might be better:
1. Put in P
2. Turn engine off
3. Apply parking brake
4. Restart when lights are amber
5. Release parking brake
6. Put in D
7. Still beat most cars off the lights
Yes, parking brake is still there for when you actually park. But the hold feature just works with the foot brake- stop as normal, then give the pedal a little extra press, and the dashboard should say something like "hold". At that point you can take your foot off the pedals. If the engine has stop-start then it may even switch itself off as well - but to get going again, you just depress the accelerator pedal, the engine starts and the brake releases. It's the newer trouble free electronic version of the old SBC pump system that has caused a few issues on the original 211 series cars (up to about 2006, iirc).
Leave it in drive, foot on the brake leaving both hands free to have a good old fashioned pick on my hooter. Then lower the window and flick the contents of my nose out like smokers do with their fag ends.
Then when the light turns green I thrash my foot on the gas, the engine revs, the wheels do not turn. I realise I'm in my manual transit van and not my auto car.

So I do have the HOLD feature! I wonder how many other owners dont know about it. Seems to be exactly what i was after. That is why i love this forum, you just have to filter out the stupid replies. Many thanks for all the comments.
Probably most of those who haven't bothered reading the driver's manual. MB have provided us with lots of useful gizmos and charged us plenty for them, I know it's not very manly to read the manual, but it's actually worth it to get the best out of your car.

2013 model E class only has half the information in the manual.
In the documents folder you get 2 discs labled 1/2 and 2/2 to load into Comand
So I thought I would try to answer a couple of questions yesterday - turns out the previous owner hadn't loaded the discs.
So I insert 1/2 and half an hour later it gets spat out. Then try to load 2/2. Five minutes later it asks me If I want to replace all the data from 1/2 with the same data from 2/2
I did manage to answer the questions though
