Mercedes Vito Limp Mode

My 2016 Vito keeps going into limp mode after engine light appears. This has happened around 13 times this year with it ALWAYS happening when driving in second gear. After driving consistently throughout the day, the van also makes screeching or vacuum noises at the end of acceleration. It has had the following checked / repaired:

• Every single sensor replaced
• DPF cleaned
• New EGR valve
• Gasket for EGR
• New intake system and air box
• Boost pressure sensor

Any ideas what the problem could be? They think it could perhaps be the actuator. Thanks!
