W245 Bclass 2006 - Surging - P0401 - Diagnose and DIY fix

My BClass (60k miles) has developed a surging issue until up to Temp and is intermittently falling into limp mode after / during motorway runs

Using a OBD reader it initially reported a confirmed error P0132 Sensor Circuit High Voltage (Bank 1 Sensor 1) and pending P0401 Exhaust Gas Recirculation Flow Insufficient Detected.

The pneumatic EGR looks relatively new to the rest of the vehicle, I have had it out, it was a bit mucky so I have cleaned, there is movement in the valve and I re-installed. I also shifted the tube between the EGR and the pressure tranducer(?) which look as though it had been slightly crushed when the airbox was refitted.

The P0132 code has not returned but I still have surging before the car heats up (and voltage is up) during the surging headlights and dash lights surge at the same time. I now only get a pending P0401 fault on the reader, the freeze frame info is as follows.

MAF Airflow – 54.91g/sec
MAP – 148.00 kPA
RPM – 2357
Intake Air pressure – 11.c
Coolant temp – 75c
Engine Load 50.2%

I also checked the dynamic parameters whilst driving – O2S1 (Bank 1) was reading between 1.16 and 2.00v ( its steady 2v at idle when warm not sure whether it is right or not?) O2S1 (Bank 2) reads between 0.00 – 0.01% again not sure this is right / important or relevant.

Voltage on the vehicle when I get in is around 11.2-11.6 v – which rises to a maximum of 14.1v after a 10 mile run.

Any help on diagnosing and DIY fixes gratefully received!
 
Although I have a B180 Cdi W245 I know little about these engines, but it does sound like a cold start valve maybe dirty and need cleaning, if there is one fitted to diesel engines. Not much help probably.
 
If there is one - so little information on this vehicle. Just going to clean as much as I can and see where it gets me - does anyone know where the throttle body is located. Can probably do a bit of trial and error but any guidance would be great
 
Your B245.208 has the OM640.941 DIESEL ENGINE. As you say there's very little info out there on it.

If automatic the car has CVT autobox known to give trouble.
the throttle body no varies depending on emission controls,DPF is fitted but should something like this A6400902070
