260e starting noise

I have a 1990 260e which has covered 129000 miles. From a cold start there is a clatter from the top end of the engine for 2/3 seconds during which time it also misfires. In about 1 in 10 cold starts a light tapping noise can be heard for 15/20 seconds. Once running it sounds fine. If I turn the key on and off so the engine turns for a second the problem does not occur. I use 15/40 oil which is changed regularly. Any help would be much appreciated.
 
Without hearing it it's hard to say but sounds like stuck tappets. The bit that's odd is that if you turn the engine over for a second all is ok. Hmm.

Try running some flushing oil through the engine and see if that releases the tappets, otherwise they need to come out for inspection and possible replacement.

The only other thing I can think of for this is the cam chain tensioner is stuck off load. Perhaps check that and maybe fit a new one to be sure.
