I'm not sure that my 320 cdi does either - certainly cannot find a menu for it. I have a theory that to jam the 3.0 engine in they ditched the heater. No idea if it is correct though.
 
In my w203 saloon the first aid kit is tucked under a sort of speaker grillle (that what i can describe it best as) on the parcel shelf.
It has a green circle with a white cross as far as i can remember on the catch that opens it to reveal a bag full of goodies, my only thought is, i hope i never need to use it for obvious reasons.

Why i ask myself is there i not some sort of notice stuck onto the sunvisor showing you where this is placed in all our mercs, for some owners it seems like an episode of the crystal maze trying to find something you will most likely only need in an emergency. Wheres the logic in that.
 
The handbook for my new Vito said the first aid kit (and warning triangle) were inside one of the front doors, but they're not. In fact there's no storage compartment there at all?!

In the Viano the first aid kit is in the front door pocket. Just a guess but look in the storage compartments under the front seats where the towing eyes etc are kept or the ones on either side of the boot compartment with the jack and wheelbrace.
 
First aid kits are compulsory in Germany and have been a standard fit on Mercedes for years for that reason. They were always placed on the centre of the rear parcel shelf where they:-
1. could easily be seen from outside the car.
and
2.where they were probably most accessible after a crash

Unfortunately changes in body style has dictated that this "standard" position has changed with current kits hidden away in compartments in the boot (albeit labelled) or under the front seat in some models ( fire extinguisher under the other) and have become less obvious. Warning triangle
The warning triangle is located in the stowage compartment in the driver’s door.

First-aid kit
The first-aid kit is located in the stowage compartment in the co-driver’s door.
Unfortunately neither door has a "stowage compartment". They just have an open pocket, with a bottle holder (1) at the front:
