Don't think Miltek do exhausts for Mercedes...

They're more VAG-oriented.

You could try brabus, kleemann perhaps.

I looked a while ago and came to the conclusion that options were limited - you're either looking at one of the fairly expensive aftermarket tuners or you're left with getting a system built at the likes of Powerflow (and the results you get will depend entirely on the franchise you use, and how awake the guy building your exhaust is).

Hopefully someone will come along and tell us there are better options, but I've not seen any yet.
