I have an 04 w211 - the radio reception is really rubbish. If i'm driving along, it can hold a station for a little while but then it fuzzes out and i have to switch. What cables do i need to follow and what boxes do i need to check? Could it be as simple as a loose aerial lead at the back of the head unit?

Not an expert but I think it may be the booster that has failed(If you have one) Mine had a similar problem and I had to replace a fuse in the boot that fixed it.

My similar problem was the aerial plug at the back of Comand. One of the little spring fingers was bent and shorted to the centre pin. I had spent time and money changing the amp behind the rear pillar without improving reception.
Good luck

Hmm - so might make sense to a) check fuses and b) check the back of the head unit (just audio 20)

There will be a fuse in the boot area of the car , to the left for the aeril amp . The amp also boost the central locking range , and if the fuse checks out ok and the cental locking range is not good either than its probably worth changing the amp , there around 180 pound
