Is the variable cam timing thing causing this?
It sounds like a knackered roller bearing to me. it could be the belt tensioner.

You probably need a new one. They are bolted to the end of the cam. They are mega money iirc.
the variable timing adjuster... but are you guys sure nothing else is adrift before you go spending money?
